Yes, it does. As a statement, it is answering the question it looks
like with a "yes".

So: "Have you had a nice day?"
"Have I ever!" - means I've had an extremely nice day.

Yes I'd say it's pretty informal.
It sounds like US English to me, not the sort of thing you'd expect a
British English speaker to say (possibly because GB English speakers
generally go in for understatement, not enthusiastic outpourings of emotion
[and "Do I ever!" sounds *very* enthusiastic]).
It also sounds a little dated and/or provincial to my English ears -- a bit
Waltonsesque, harking back to more innocent times.
I could be wrong, but that's the way it comes across to me. (And context of
course counts for a lot. It's possible that in other contexts "Do I ever!"
might sound perfectly normal to me).

Here's a context I just thought up:
Person 1: "Do you like comedies?" (i.e. Do you like comedy films?)
Person 2: "Do I ever!"

"Translating" that into GB English you might get the following understated
reply:
Person 1: "Do you like comedies?"
Person 2: "Yeah, they're OK" [which you might hear even if Person 2 *loves*
comedies! It's a British thing!] Surprised)
