Is it possible to pass a two language reading proficiency exam or a one
language high-proficiency exam for, say, a doctorate in literature or
comp lit or English or history, if you have only one or two years of
high school Spanish taken 20 years ago? Should I look for a program
where no such requirement exists or figure out a plan to get up to
speed in the language either before I get into a program or after?

It's totally possible to pass with that. It tends to mean a "reading
knowledge" which can usually be fulfilled with either a 3 credit class
or a really simple test. i just did one for French after not having
French since high school. I'd try to get up to speed, but you don't
need to know *that* much that it's an insurmountable obstacle.
