Suppose Joe buys a cup of coffee from Jane. Joe doesn't like it etc...

The question is: given that Jane gives Joe a refund, is the cup of coffee

1. refunded by Jane

or

2. refunded from Jane's Company

or are both correct? What is the difference between "refunded by"
and "refunded from". Is it a matter of person vs. company or what?
Or are they 100% interchangeable. Or is one of them plain wrong.
I just can't make it out.

"Refunded by" is correct here (better: it's the COST of the cup of
coffee which is refunded).

You could say that the cost of the cup of coffee was refunded by Jane
from the cash-till.

The cup of coffee isn't refunded. The money is refunded (to Joe, by
Jane, from Jane's company); the coffee is returned (by Joe, to Jane).

The passive voice is problematic here. Shift to active and you'll see:

Joe returned the coffee to Jane, and Jane refunded Joe's money.
