Please tell me again how I'm supposed to punctuate the end of these
sentences.
Before the Tonight Show he was host of "Do you trust your wife?".
After a couple years its name was changed to "Who do you trust?".

Here's my take --
Before the Tonight Show, he hosted "Do You Trust Your Wife?".
After a couple years, its name became "Who Do You Trust?".
I prefer a single sentence though --
Before the Tonight Show, he hosted "Do You Trust Your Wife?",
which became "Who Do You Trust?" after a couple years.

Those look good to me.
However, _The Perrin-Smith Handbook of Current English_ has these
examples of correct use:
"Why do you say that?" Mother asked me.
He gave me a skeptical look which seemed to mean "Look who's
talking!" [No final period]
Did you hear him say, "Are you crazy?" [a second question mark would
violate their "no doubled punctuation" rule]
A parallel to the last, from _Harbrace College Handbook_, but without
the double question, is
He asked, "When did you arrive?" [No final period]
You example, of course, is quite different, being of a title rather than
direct (or ascribed) speech.
Kate Turabian's _A Manual for Writers_, a revision of the
_Chicago Manual of Style_ for typewritten papers, has no examples of
exceptions (in pars. 3:107-108, 5:14, 5:28-29) to the general rule that
a final '?' within a quotation is not followed with another punctuation
mark.

But, of course, isn't it "whom do you trust?"

They look fine to me as they are, but that's a Rightpondian view -- I
believe the most common AmEng style is to omit the final full stop.
(Haven't we done this many, many times? It must be in the FAQ.)
